No. Pillar Healthspan is not insurance and doesn't replace it. Your membership covers your relationship with Dr. Sadler — direct access, comprehensive primary care, and preventive planning. You'll still want insurance (or Medicare) in place for hospital care, specialist visits, imaging, medications, and emergencies. Many members pair their Pillar Healthspan membership with a high-deductible health plan for exactly this reason.
For many members, yes. As of January 1, 2026, federal law recognizes qualifying direct primary care memberships as HSA-eligible up to $150/month for an individual — exactly where our membership is priced. Because eligibility depends on your specific health plan, we recommend confirming with your HSA administrator or tax advisor before enrolling, particularly regarding the one-time initiation fee.
Pillar Healthspan membership fees are paid directly and are not billed to Medicare or Medicaid. Many members keep their existing Medicare or Medicaid coverage active for hospital care and specialist referrals alongside their membership.
Always use your insurance and, in an emergency, call 911 or go to the nearest emergency room. Membership doesn't cover hospital or emergency services — it's what happens around them: helping you get to the right specialist, coordinating your care, and making sure nothing falls through the cracks.
The HRI is included in every new member's one-time initiation. Establishing your physiologic baseline — strength, cardiorespiratory fitness, and key biomarkers — is core to how we practice medicine here, not an optional add-on.
Plan for about an hour. The cardiorespiratory and strength testing are submaximal — built to estimate your fitness without requiring you to push to exhaustion — and the lab draw is a standard blood draw. Most members find it closer to a solid workout than a medical procedure.
Dr. Sadler is a board-certified family medicine physician with a background in primary care, medical leadership, and applied microeconomics. He founded Pillar Healthspan to create a more proactive, evidence-based model of medicine focused on helping patients maximize long-term health, resilience, and quality of life.
Throughout his career, Dr. Sadler has cared for diverse patient populations across military medicine, veteran healthcare, and community primary care while also serving in physician leadership roles overseeing clinical operations, quality standards, and physician training. His experience includes work in preventive medicine, sports medicine, operational medicine, and longevity-focused biomarker analysis.
At Pillar Healthspan, his philosophy centers on the belief that the greatest drivers of long-term health are often the most foundational: exercise, nutrition, sleep, and meaningful relationships. Rather than reacting to disease after it develops, he works closely with patients to strengthen physiologic reserve, improve metabolic and cardiovascular health, and preserve long-term function and independence.
